We are thankful that you have granted us permission to act as the dispatching provider for your company, covering your truck(s) and providing administrative (back office) support. We value your business and will prove it with every task.

To get setup, please complete and return the following items as detailed below.

• Carrier Profile – (located on our website – click here)

• Limited Power of Attorney – (located on our website - click here)

• Copy of your Carrier’s MC Authority – email to info@258logistics.com or UPLOAD below

• Copy of your W-9 – email to info@258logistics.com or UPLOAD below.

• Copy of your certificate of Insurance (COI) – email to info@258logistics.com or UPLOAD BELOW

We require Cargo and Liability in the amount required by the respective broker.
